Welcome to the Condition Management Programme (CMP), part of Pathways to Work in Lancashire. This website is designed to inform visitors about the Condition Management Programme; to share information relating to the programme; to provide answers to your questions and offer a forum for discussion and debate.

The Condition Management Programme is a short health focussed programme aimed at supporting those people who are in receipt of Incapacity Benefit to manage their long-term conditions in order to return to work. The programme is delivered by the NHS, but is a partnership between the NHS and Jobcentre Plus.

The longer a person is in receipt of benefit the less likely their return to work, and the less likely they are to manage or improve their conditions.
